In the joint divorce application the mutual agreement must be approved and the following information must be mentioned:
a) Name and surname of each spouse. Birthplace and date of birth, nationality, living place, education, working place and occupation
b) The data of registration of the act of marriage
c) Last names that the spouses choose after the divorce
d) The data of identification documents of the spouse
e) Other necessary data
