Due to the epidemic in the Republic of Armenia, the Ministry of Health of the Republic of Armenia temporarily suspends receiving paper applications and letters from citizens, individual entrepreneurs, organizations.

The Ministry of Health reports.

The application/letter should be submitted electronically via the official e-mail address of the Ministry of Justice of the Republic of Armenia ([email protected]) or via the Unified E-Query Platform (https://e-request.am/hy/e-letter), filling in the required fields.

Application forms can be downloaded at http://moh.am/#1/199. The application must be signed and include the applicant’s name, surname, address and/or e-mail address, telephone number.
