The Civil Status Acts Registration Agency compiles and provides to the parents (and/or one of them) the certificate of determination of paternity, based on their application.
The certificate of determination of paternity includes the following information:

1. Fisrt name, last name and middle name of the person who is identified as father, date and place of his birth, nationality-in accordance to the note of determination of paternity.

2. Fisrt name, last name and middle name of the child before and after determination of paternity, date and place of his/her birth.

3. Fisrt name, last name and middle name of the child’s mother, date and place of her birth, nationality-in accordance to the note of determination of paternity.

4. Date and number of the note of determination of paternity.

5. Place of the note of determination of paternity (The name of Agency of registration of civil status acts).

6. Date of issuing  the certificate of determination of paternity.
